Arizona Chapter is augmenting the Awards program with three new prestigious awards in recognition of members that have made substantial contributions to the Arizona Chapter of HFMA and the healthcare financial management profession. These recognitions will be awarded annually. Members will be recognized based on nominations from other HFMA members.

All nominations will be reviewed by the Awards Committee and recommendations will be presented to Board of Directors for approval. The selected members will be honored at the Spring Conference each year and presented with an engraved glass plaque award.

Criteria for the awards:
> The recipient must be an HFMA member.
> Both Providers and Vendors will be recognized.
> Awards will be given annually.
> Three recipients will be awarded annually (at least two providers and at least one vendor)

HFMA - Arizona Chapter - Lifetime Achievement Award
The professional selected for this category will be an HFMA member that has invested 20 years or more in the business of caring with consistent dedication. This individual is highly recognized in Arizona for making a positive impact difference in the healthcare financial management profession in Arizona.

HFMA - Arizona Chapter - Member of the Year Award
The professional selected for this category will be an individual that has made a substantial contribution to HFMA - Arizona Chapter as demonstrated by four criteria.

HFMA - Arizona Chapter - Diamond Inspiration Award - Leadership Award
The healthcare financial professional selected for this category will be an individual that is recognized for leadership and inspiration in Arizona based on four criteria.
